Absentee Ballot Application


 

Voter Eligibility
You must be:
18 years of age
US Citizen
Resident in the district for 30 days preceding May 19, 2009

Voting by absentee ballot is a two-step process.
  1. The application for an absentee ballot which appears on this website can be downloaded and printed. It must be filled out in its entirety and returned with the original signature to the Office of the District Clerk, 100 Campus Drive, Port Washington NY by May 12th if the ballot is to be mailed to the voter, or at least the day before the election if the ballot will be picked up by/for the voter.

  2. Once the application is received and verified, a ballot will be forwarded to the registered voter. The sealed ballot must be returned by mail or hand delivered by another individual for the absentee voter, to be received no later then 5:00 p.m. on election day, Tuesday, May 19, 2009.
